The pain of labour is a daunting prospect for many mums-to-be. And while labour is a unique experience for every woman, varying in levels of pain and intensity, the more informed you are about the progress of labour pain and the choices of pain relief available, the more prepared you’ll feel in the lead-up to birth.

Once you’re in labour, you’ll likely be receiving the emotional and physical support you need from your midwife, doula, and/or partner, but you may also need the added support of pain relief to help you cope.

Your midwife will support you through labour with natural methods such as breathing exercises, and if you would like stronger pain relief, they will be able to advise you on what’s available, such as analgesics or an epidural.

In current practice, 70–80% of first-time mothers opt for an epidural in labour.

What pain relief is available during each stage of labour?

Early first stage

During this stage, the cervix starts to dilate, and you will experience mild contractions. Natural methods such as massage and breathing techniques can be helpful, but if you need stronger pain relief, you will be offered analgesics such as pethidine, which dulls the pain but allows you to remain active.

Active first stage

During this stage, the cervix will dilate more quickly, and the contractions will be stronger and more frequent. While some women are OK to carry on with natural or analgesic pain relief, others may require stronger relief, such as an epidural.

Transition

As the cervix dilates fully, contractions become a lot more intense. At this stage, Entonox is effective pain relief that does not impact the baby. If you have chosen to have an epidural, this can be topped up.

Second and third stages

The second stage starts with full dilation of the cervix and lasts right up to birth. Contractions are intense and longer-lasting, but easier to deal with as you start to push. You may wish to use gas and air at this stage. In the third stage, the placenta is delivered, contractions subside, and you should not need any pain relief.
